'Cardiologists in NJ scoring under 75' = state NJ + specialty cardiology + max_final_score 74.99. Specialty is matched to CMS's spellings for the year (e.g. 'cardiology' -> 'Physician/Cardiovascular Disease (Cardiology)' plus the interventional and heart-failure cardiology values)."
